The Life insurance benefit will be paid to your designated beneficiary in the event of death while covered under the plan. The benefits described below will begin to decrease at age 65. Contact Guardian directly for conversion and/or portability options.
Evidence of Insurability (EOI) is required for any election or increase made after initial offering of benefit, as well as any amount over the GI. Once the election request is complete, an EOI is sent via email to the employee to complete. Coverage will not begin until EOI has been reviewed and approved by Guardian.
